

Senior Physician Opportunity
Boston Senior Medicine is seeking an Internal Medicine/Primary Care MD/DO for homebound patient visits. Our medical practice is patient focused and our providers can take special satisfaction with knowing we provide them with the support staff and encouragement to build relationships with their patients. We see patients in their homes, so our patients are neighbors and friends, not numbers and data files. Our PCPs provide the full scope of primary care services, including diagnosis, treatment, coordination of care, preventative care and health maintenance to patients. Our office team is an excellent support staff and handles all of the administrative burden, which allows our providers to focus on our top priority of patient care. They serve as leaders within the patient centered medical home model and work collaboratively with our multidisciplinary care team to ensure that each patient gets the best possible care. Patient volume is approximately 20 patients per day, and our medical assistant will drive and assist with all patient visits. This position requires no weekend coverage or on call. All providers must have a current, unrestricted medical license in MA and have completed a medical residency program at an accredited program.
Qualifications

Completed residency in Internal or Family Medicine, with a fellowship in Geriatrics strongly preferred

Active and unrestricted Massachusetts medical license

Board Certified or Board Eligible preferred, but not required

ACLS and BLS certification required

Clinical excellence in evidence-based geriatric care

The ability to confidently navigate uncertain situations with both patients, families and colleagues

The ability to adapt to personal and interpersonal behavior to meet the needs of our patients

The ability to effectively communicate with patients, their families and provide care to geriatric patients of all backgrounds
Essential Duties and Responsibilities

Participates as an active member of the medical staff and participates in daily care team huddles to ensure prioritization of patient needs.

Attends to patients in a timely and orderly manner to keep waiting time to a minimum and maximize productivity.

Obtains appropriate patient history, performs physical evaluation, orders appropriate diagnostic tests and follows-up with interpretation, formulates assessment and initiates treatment plan.

Reviews incoming reports (e.g. lab, x-ray, EKG) signs, dates and follows up with patients in a timely manner.

Ensures documentation is recorded in a complete, timely and orderly manner, utilizing and completing each part of the written templates and template in the electronic medical record, signing the paper sheet before the end of the business day and signing the electronic documentation in the EMR within 24 hours of transcription.

Adheres to BSM productivity and quality standards and meets minimum requirements.

Consults and refers with/to appropriate Specialty Physicians.

Participates in the quality care review and chart review program.

Assesses, stabilizes and triages patients as appropriate.

They serve as leaders within the patient centered medical home model and work collaboratively with our multidisciplinary care team to ensure that each patient gets the best possible care. Qualifications

Masters Degree as a Family Nurse Practitioner, geriatrics experience preferred

At least two years working as an NP, with the ability to operate independently
